Credit Card on File

All patients must keep a valid credit/debit card on file in our secure, compliant system.

• Claims: We submit to insurance first.
• Notification: Once your insurer determines your portion (co-insurance, deductible, uncovered services), we will notify you of the balance.
• 10-Day Grace: You have 10 days to pay by alternate means or contact us with concerns.
• Automatic Charge: After 10 days, we will charge the card on file for any outstanding balance.

Cancellations / No-Shows

We attempt to confirm appointments via phone, email, or text. If you must cancel or reschedule, please give at least 24 hours’ notice. Failure to do so or a missed visit will incur a $50 no-show fee, charged to the card on file.

Patient Billing

If you still owe a balance after insurance processes your claim, you may receive up to three statements. If unpaid, your account may be sent to collections, potentially with additional fees. Returned checks incur a $25 fee. If your insurer sends you payment directly, please forward it to us.

Patient Agreement & Assignment of Benefits

I have read and understand these policies. I agree to pay any balance not covered by my insurance(s) and to keep a credit card on file as outlined above. I assign benefits to Sharma Foot & Ankle Specialist for services rendered and authorize the release of medical information necessary for claim processing.

Insurance

We participate in most insurance plans. If we do not participate in your plan or if you cannot present an up-to-date insurance card, payment in full is required at each visit until coverage is verified. Knowing your insurance benefits (including referrals) is your responsibility. We will submit claims on your behalf, but any balance is ultimately yours to pay.

Medicare

As a participating provider, we will bill Medicare and any secondary insurance. Not all services are covered; you remain responsible for deductibles, co-insurance, or non-covered items.

Copays, Deductibles & Self-Pay

All copayments and deductibles must be paid at the time of service. Self-pay patients must pay in full at each visit. Some services may be deemed non-covered by insurers, and you are responsible for those charges.
